To Our Residents, Family Members, and Responsible Parties:

We hope you are staying safe and healthy. As part of our goal to be transparent about how Covid-19 is affecting us here at A Grace Sub Acute & Skilled Care, we will continue to provide updates on this webpage.

  • We are currently performing regular Covid-19 testing for staff and residents as needed. Frequency may change depending on guidance from Santa Clara Public Health Department. Staff members (including vendors and approved visitors) are also screened for any respiratory symptoms, fever, or possible exposure upon entry. Those who show symptoms are instructed to go home until they meet Santa Clara County Public Health's "Return-to-work" criteria.
  • Visitation must be arranged prior to the planned date of visit, so we can ensure proper measures are taken to ensure the safety of our residents and staff. We are also offering "Facetime/Zoom" visits and other forms of virtual visits via iPads that we've provided for our residents and our Activities Department. As an alternative, we are coordinating "window" visits and outdoor visits scheduled in advance. Guidance changes depending on what "tier" Santa Clara County is currently in.
  • Covid-19 Vaccination Update.  We successfully completed vaccination of current residents and staff at the facility and we are still vaccinating new residents and staff as requested.  If you want your family member who is a resident at the facility to get vaccinated, please contact us for details.
  • Covid-19 Case Update.  As of August 4, 2022 we have 2 reported case of Covid-19 positive among our staff and all residents remains negative.  We continue to follow the CDC and CDPH guidelines and are screening all visitors in the facility.
